Praise & Prayer Request

We have 3 type of praise & prayer request available.  Please read the descriptions and pick the one you would like to use and you will be taken to the praise & prayer request form for that catagory.

Public Request   Upon completion of this praise & prayer request form it will be submitted for review by a member of our staff.  After approval the request will be broadcast on the CCM prayer chain network and it will also be added to the public praise & prayer chain section displayed on the CCM website.
Elder Request   Upon completion, this praise & prayer request will be transmitted directly to the CCM Elders and our Pastor ONLY.
Pastor Only Request   Upon completion, this praise & prayer request will be transmitted directly to the our Pastor and ONLY our Pastor.
